Resumo: Female patient, 63 years old, diagnosed with Systemic Lupus Erythematosus, Chronic Kidney Disease, Rheumatoid Arthritis, and secondary Sjögrens Syndrome. She had been using methotrexate weekly for the past three months to control rheumatic disease. She was admitted to the Emergency Department with swallowing difficulty for one week, progressing to dysphagia and airway protection failure, requiring mechanical ventilation and being referred to the ICU. In the intraoral examination, multiple ulcerated and erosive lesions were found in the oral and labial mucosa, with complaints of pain upon manipulation and spontaneous bleeding. Hemogram showed severe pancytopenia, which was a determining factor for the final diagnosis of methotrexate toxicity and oral lesions due to drug-induced stomatotoxicity. The lesions were treated with 20 sessions of photobiomodulation, with daily application, and oral hygiene protocol with 0.12% chlorhexidine, showing total regression of oral lesions.
