Resumo: Ameloblastic fibroma (AF) is a rare benign mixed odontogenic neoplasm that can occur in the mandible or maxilla but is predominantly found in the posterior region of the mandible. Most AF cases occur in the first two decades of life and primarily affect male patients. Here we report the case of a 16-year-old female patient with an expansive radiolucent lesion rupturing the cortical region located in the left maxilla. Excision was performed, and histopathological analysis revealed a mixed odontogenic neoplasm composed of islands and strands of epithelial cells in a loose connective tissue stroma with a myxomatous appearance. Epithelial cells were arranged in small nests with scant cytoplasm and basophilic nuclei, and cystic areas with ghost cells were also observed. A cell proliferation index measured by Ki-67 was approximately 1%. The clinical, radiographic, and microscopic features favored the diagnosis of AF with atypical mesenchymal cellular areas.
