Resumo: A 50-year-old man presented a 6-year evolution asymptomatic lesion on the midline of the hard palate. Physical examination revealed a pedunculated nodule, 2 cm in diameter, with a smooth surface, normal coloring, and firm consistency. The patient reported a successfully treated Hodgkins lymphoma at age 27 and current heart disease. Excisional biopsy was performed, considering benign glandular neoplasms and inflammatory fibrous hyperplasia as diagnostic hypotheses. Histopathologic examination revealed a mesenchymal neoplasm with proliferation of multiple vessels with thick walls. Immunohistochemical staining with SMA (smooth muscle actin) and CD31 (endothelial cells marker) guided to establish the diagnosis of angioleiomyoma. Ki67 and S100 markers were also used to exclude malignancy or alternative lesions. Oral angioleiomyoma is a rare lesion, with surgical excision being the treatment of choice. Supplementary exams are crucial for differential diagnosis from other benign lesions. The patient remains under clinical follow-up and no recurrence was detected after 6 months.
