Título: MUCOCUTANEOUS LICHENOID REACTION ASSOCIATED WITH PEMBROLIZUMAB IN A PATIENT WITH PROSTATE CANCER: A CASE REPORT

Resumo: A 71-year-old man, diagnosed with prostate cancer in 2013, underwent prostatectomy followed by radiotherapy and goserelin. After 6-year, high serum prostate-specific antigen levels and iliac lymphadenomegaly were noted. Pembrolizumab combined with enzalutamide was initiated. After 8 months, erythematous papules with scales appeared on the extremities, alongside white striated lesions on the bilateral buccal mucosa, tongue, and vermilion and lower inner lip. Biopsies of the skin and oral mucosa revealed a dense band-like infiltrate of lymphocytes. Mometasone furoate was prescribed to treat the skin lesions, while clobetasol propionate 0.05% ointment was administered three times daily for the oral lesions. Improvement of both sites was observed. However, due to high level of transaminases at the 19th cycle, pembrolizumab was discontinued. High-dose oral corticosteroids were initiated to manage the liver immunotoxicity. Mucocutaneous immune-related adverse events persisted even after discontinuation of pembrolizumab therapy. The patient remains under clinical follow-up reporting only minor symptoms.
