Título: MULTIMODAL TREATMENT OF MEDICATION-RELATED OSTEONECROSIS OF THE JAW: CILOSTAZOL, TOCOPHEROL, AND PHOTODYNAMIC THERAPY

Resumo: A 63-year-old woman with a history of breast cancer, undergoing zoledronic acid treatment for osteoporosis every six months, reported pain in the anterior maxillary area following dental extractions nine months prior. The intraoral examination showed exposed necrotic bone and purulent drainage in the anterior maxillary region. Computed tomography displayed alterations in the bone trabecular pattern. The diagnosis of medication-related osteonecrosis of the jaw was established, and initial treatment included antibiotic therapy alongside pentoxifylline and tocopherol, both at 400 mg twice daily. Due to severe nausea, pentoxifylline was replaced with cilostazol 100mg twice daily. Additionally, weekly applications of antimicrobial photodynamic therapy (methylene blue 0.01%; 660 nm; 100 mW, 6J) was performed. After 2 months, sequestration of the exposed necrotic bone was observed, and complete wound healing occurred after an additional 2 weeks. No surgical management was necessary and the patient remains in clinical follow-up.
