Resumo: Peripheral ossifying fibroma (POF) is a reactive lesion that occurs more frequently in children and women in the anterior region of the maxilla. Typically, these lesions occur in 2% of all gingival biopsies. A case of a 3-year-old male leukoderma patient is presented, who began to experience generalized sessile gingival growth, with lesions covering all teeth. General physical examination revealed short stature, while intraoral examination showed painless, pink-colored fibroelastic lesions covering all teeth, with no radiographic alterations. The first intervention was performed at the age of 3 under general anesthesia in 2002, with a diagnosis of peripheral ossifying fibroma. Subsequently, there were several recurrences. Numerous examinations, including genetic ones, were performed, and over the course of 20 years, management strategies were discussed in light of more than 50 biopsies diagnosed as POF. Treatment involved multiple extractions and placement of two protocols of osseointegrated implants.
