Resumo: The case report involves a 35-year-old male patient diagnosed with pleomorphic adenoma in the hard palate region. The treatment consisted in lesion ablation under general anesthesia. After 17 years, the patient presented a carcinoma ex pleomorphic adenoma diagnosed by incisional biopsy. Tomography evaluation showed extensive bone destruction affecting part of the nasal cavity and maxillary sinus.
The patient underwent maxillectomy, resulting in a significant palatal defect communicating with the maxillary sinus and nasal cavity, impacting speech intelligibility, necessitating nasogastric tube feeding, drastically compromising the quality of life. For rehabilitation, the manufacturing of a bucomaxillofacial prosthesis was chosen, using a palatal obturator adapted to the patients upper total prosthesis, resulting in adequate social reintegration and significant improvement in quality of life, eliminating enteral nutrition and enabling satisfactory communication.
