Resumo: Aneurysmal bone cysts (ABCs) are locally aggressive bone tumors mainly affect pediatric patients. ABCs comprise about 2% of all primary bone tumors. Males and females are equally affected, with uniform skeletal distribution. Previous findings of immunohistochemical studies, through a limited number of vascular markers, support a vascular lining of the cavities. Case 1: 15-year-old boy presented an expansive osteolytic lesion on right posterior mandible, of rapid evolution. After biopsy, the diagnosis was ABC. Case 2: 25-year-old male presented with an asymptomatic tumor on the left posterior mandible, showing evident facial asymmetry, several years ago. Biopsy confirmed the ABC diagnosis. In both cases, 1-year after surgery, there are no alterations. By immunohistochemistry, using CD31, CD34, CD105, ERG, D2-40, FLI-1, GLUT-1 (vascular markers); α-SMA, calponin, h-caldesmon (stromal markers), CD68 (macrophages), and Ki-67, revealed that most of these markers were restricted to fibrous septa, sparing the lining of blood-filled cystic spaces.
