Resumo: Vermilionectomy is the ultimate treatment for actinic cheilitis (AC) showing severe epithelial dysplasia. Herein, we report a case of a 40-year old male presenting white plaques and erosion areas throughout the lower lip vermillion. The patient reported a long history of solar exposure during your lifetime, especially due to labor. Initially, an incisional biopsy was performed and histopathological analysis established a severe dysplasia AC diagnosis. Therefore, with the present degree of epithelial dysplasia, vermilionectomy was performed under local anesthesia and conscious sedation. An hour before the procedure, it was orally prescribed 15 mg of midazolam, 2g of amoxicillin and 8 mg of dexamethasone. Immediately after local anesthesia, the total extension of the lower lip vermillion was excised using scalpel and electrocautery. Special care was provided to hemostasia and the inferior labial mucosa was advanced and interrupted 4-0 nylon suture. Patient recovery was uneventful and follow-up showed excellent aesthetic results.
