Resumo: Objective: To characterize a cohort of young adults with oral squamous cell carcinoma (OSCC), considering clinicopathological and prognostic variables.

Methods: A multicentric retrospective study was conducted on patients diagnosed with OSCC under 45 years old between 2009 and 2023 from four diagnostic centers in Argentina (Buenos Aires, Córdoba, Salta, and Chubut).

Results: Seventy-two patients were included, with an average age of 37.77 years, comprising 51 men (70%) and 21 women (30%). The lateral tongue border was the most frequent location (n=48; 66.2%). Ulcerated OSCC was the most common clinical phenotype (27; 37.5%). OSCC was not associated with oral potentially malignant disorders (OPMD) in 54 cases (75%). Systemic comorbidities were found in 10 patients (14%), most of which were associated with immunosuppression subtypes. Regarding histological variables, moderately-differentiated OSCC (48.6%) was the most prevalent. At the initial consultation, 41 (57%) individuals were diagnosed at late stages (T3, T4). Despite more than 70% missing data in this variable, only 7 patients were recorded as alive at 5 years of follow-up.

Conclusion: Despite variations compared to previous studies, OSCC in this Argentine subgroup of young adults exhibits well-defined clinical patterns and behaves as a highly aggressive entity diagnosed at a late stage.
