Resumo: Objective: To characterize a cohort of young adults with oral squamous cell carcinoma (OSCC), considering clinicopathological and prognostic variables.Methods: A multicentric retrospective study was conducted on patients diagnosed with OSCC under 45 years old between 2009 and 2023 from four diagnostic centers in Argentina (Buenos Aires, Córdoba, Salta, and Chubut).Results: Seventy-two patients were included, with an average age of 37.77 years, comprising 51 men (70%) and 21 women (30%). The lateral tongue border was the most frequent location (n=48; 66.2%). Ulcerated OSCC was the most common clinical phenotype (27; 37.5%). OSCC was not associated with oral potentially malignant disorders (OPMD) in 54 cases (75%). Systemic comorbidities were found in 10 patients (14%), most of which were associated with immunosuppression subtypes. Regarding histological variables, moderately-differentiated OSCC (48.6%) was the most prevalent. At the initial consultation, 41 (57%) individuals were diagnosed at late stages (T3, T4). Despite more than 70% missing data in this variable, only 7 patients were recorded as alive at 5 years of follow-up.Conclusion: Despite variations compared to previous studies, OSCC in this Argentine subgroup of young adults exhibits well-defined clinical patterns and behaves as a highly aggressive entity diagnosed at a late stage.
