Resumo: A 49-year-old Caucasian female patient sought dental care for an evaluation of a previously performed implant in the anterior region of the maxilla. A panoramic X-ray was requested, revealing an extensive, rounded radiolucent lesion with well-defined margins in the edentulous region of the mandible body on the left side. She reported dental extractions at sites 35 and 36, 30 years ago. Intraoral clinical examination showed no bone expansion. The diagnostic hypotheses were residual cyst and odontogenic keratocyst. An incisional biopsy was performed, and after histopathological and immunohistochemical examination (S100), the report was compatible with intraosseous schwannoma. Based on this diagnosis, total removal of the lesion was planned. This case report demonstrates that intraosseous lesions may present similar radiographic images, which may surprise professionals during the diagnostic process. Therefore, professionals should always refer the removed lesions for histopathological investigation, even in the absence of suspicion of malignancy.
