Título: Cutaneous and oral adverse events associated with PD-1 inhibitors: clinicopathological and immunohistochemical aspects

Resumo: Objective: The aim of this study was to evaluate the clinical, histopathological, and immunohistochemical features of cutaneous and oral immune-related adverse events (irAEs) associated with immunotherapy with programmed cell death protein 1 (PD-1) inhibitors.
Study Design: Cancer patients who used PD-1 inhibitor immunotherapy and underwent a biopsy procedure for skin or oral lesions in an oncology hospital between September 2019 and August 2022 were included. Data regarding demographics, oncological treatment, and clinicopathological aspects were extracted from electronic medical records. Histopathological characterization and immunohistochemical assessment for CD3, CD4, CD8, CD20, CD30, and CD68 markers were performed.
Results: Seven participants who were treated with pembrolizumab (n = 5) or nivolumab (n = 2) were included. Three patients had lichenoid reactions. Sarcoid-like reaction, erythema multiforme, psoriasis, and Stevens-Johnson syndrome were also documented in one patient each. The most common histopathological characteristics included chronic inflammatory infiltrate adjacent to the surface epithelium and the presence of apoptotic keratinocytes. High immunoreactivity was noted for CD3, CD4, and CD68. All cases were negative for the CD30 antibody.
Conclusion: The majority of mucocutaneous irAEs comprised lichenoid reactions, characterized by a band-like lymphocytic inflammatory infiltrate and positive immunoreactivity for CD3 and CD4.
