Título: Immunnohistochemical expression of early carcinogenesis biomarkers in Non-Healing Chronic Traumatic Ulcers

Resumo: Objective: To describe immunostaining patterns of oral-carcinogenesis biomarkers in Non-healing Chronic Traumatic Ulcers (NHCTUs).Methods: NHCTUs of the lateral tongue were included. The immunohistochemical (IHC) study was conducted using p53, p63, Ki67, CyclinD1, and Bcl2. IHC expression was categorized into Negative, Basal, Parabasal, Suprabasal, and Full-thickness Epithelium.Results: 20 NHCTUs were included, with a mean age of 67 years with 75% of females and 25% of males. Regarding IHC expression, p63 showed the highest immunoexpression, with 40% (n=8) of cases exhibiting complete epithelial positivity, 30% (n=6) with suprabasal, and 15% (n=3) with parabasal labeling. Cyclin D1 demonstrated 50% (n=10) parabasal, and 40% (n=8) suprabasal expression. P53 exhibited 25% (n=5) parabasal, 20% (n=4) suprabasal expression, with the remaining 55% showing negativity or basal labeling. Ki67 predominantly displayed parabasal (n=8; 40%), basal (n=6; 30%), and suprabasal (n=4; 20%) expression. Bcl2 showed the least expression, with 80% (n=16) of negative scores. In 25% of cases (n=5), three or more biomarkers exhibited Suprabasal/Full thickness epithelium labeling scores.Conclusion: While traumatic lesions are typically regarded as reactive, a subset of NHCTUs may exhibit phenomena compatible with early carcinogenesis. These findings should be considered when assessing reactive changes in the context of traumatic conditions. NHCTUs require monitoring by clinicians.
