Resumo: Female patient, 64 years old, white, presented with a white spot on the right lateral edge of the tongue, which had been developing for 8 years after a facial accident. Denied smoking and drug use, but admitted alcohol consumption. In 2016, the lesion was diagnosed as chronic spongiotic mucositis. Three years later, mucosal fragments with chronic lichenoid inflammation and mild dysplasia were detected in the lesion. Also reported having been treated by a dermatologist with retinoic acid in 2023, but the lesion progressed. In intraoral physical examination, an extensive white plaque (3 centimeters), irregular, rough, defined margins, and symptomatic on palpation was identified. Due to the evolutionary clinical history of the lesion, the initial diagnostic hypothesis was proliferative verrucous leukoplakia. After a new biopsy, the diagnosis of squamous cell carcinoma was confirmed following anatomopathological and immunohistochemical analysis. Then, underwent glossectomy, removal of the right submandibular gland, cervical lymphadenectomy, and radiotherapy.
