Resumo: Lichen Planus (LP) is an autoimmune chronic disease with oral manifestations. An early detection and control of the condition is desirable. Herein, we report a case of a 34-year-old male referred for evaluation of an asymptomatic white plaques on the left posterior oral mucosa. The lesion was identified approximately two weeks before the present exam, during a routine consultation. Therefore, with the clinical diagnostic hypotheses of reticular lichen planus, leukoplakia and frictional keratosis, an excisional biopsy was performed. Histopathological analysis revealed acanthosis, hyperkeratosis and degeneration of the basal layer cells. The underlying connective tissue was permeated by a nonspecific chronic inflammatory process of subepithelial location, establishing oral LP diagnosis. The patients recovery was uneventful and clinical follow-up did not show symptoms worsening. Therefore, clinicians should aim for the early detection and histopathological diagnosis of oral LP, in order to monitor and prevent the progression of the disease.
