Resumo: A 41-year-old patient have presented with many oral cavity ulcers that had been progressing for four months. Clinical examination revealed painful erythematous ulcers with a yellowish bed, shallow with flat edges, as well as whitish areas on the gingival mucosa, buccal mucosa, and tongue. Pemphigus vulgaris (PV), lupus erythematosus and erythema multiforme were the clinical diagnosis hypotheses. An incisional biopsy was performed, and the microscopic examination was consistent with PV. The levels of G6PD were considered normal. Initial treatment with high dose of prednisone led to complete resolution of the lesions after 15 days. Followed treatment with prednisone lower concentration combined with dapsone and dapsone alone was successful applied during 7 years of follow-up. Currently, the patient is still under observation. An early diagnosis associated with suitable initial treatment is crucial to improve the follow-up and therefore ensure a satisfactory quality of life in patients affected by PV.
