Título: PREDICTIVE VALUES OF CELL CYCLE MEASUREMENTS ON MALIGNANT TRANSFORMATION OF ORAL LEUKOPLAKIA

Resumo: Despite the difficulty in risk assessing oral leukoplakia (OL) for malignant transformation (MT), recent strategies based on DNA content (DNAc) by flow cytometry have been consistent. Most flow cytometers are equipped with detectors at 585-620nm emission range, though current evidence is based on 661-683nm detectors, which could limit applicability to many laboratory set-ups. The objective of this study was to compare the accuracy of predicting MT of OL using the 585-620nm detector. Nuclei isolation was performed enzymatically on thick sections from 97 cases (18 underwent MT). Flow cytometry was used to establish DNA content based on propidium iodide fluorescent labeling of nuclear suspensions. Multiple logistic regression was used to establish DNAc thresholds for parameters (G1, S-phase, G2, 4cER) gathered from the 585-620nm detector. The predictive values (PV) of each marker were calculated from Kaplan-Meier curves (Log-rank p<0.05). The DNAc parameters 4cER, G2 and G1/G2 ratio were associated with MT (p<0.05). Two analogic algorithms from these findings generated positive PV of 38% (67% sensitivity and 76% specificity) and 40% (61% sensitivity and 80% specificity). DNA content by flow cytometry may reach PPV of 40% in high-risk lesions and 90% NPV in low-risk lesions.
