Resumo: The simultaneous presence of lichen sclerosus (LS) and leukoplakia at the same intraoral location is an uncommon finding. In these cases, the distinction is imperative, as leukoplakia is a potentially malignant disorder. Moreover, not all cases of LS may present typical histopathological criteria; therefore, strict clinical correlation is mandatory. A 58-year-old female patient was referred for evaluation of a white lesion, with unknown evolution time. The patient had been wearing complete dentures for several years. Her medical history was unremarkable. Intraoral examination showed an asymptomatic, large whitish plaque on the floor of the mouth, smooth surface, measuring approximately 5 cm in its largest diameter and, notably, a focal area with a more whitish color, adjacent to the right alveolar ridge, with a verrucous surface. After incisional biopsy, a diagnosis of verruciform hyperplasia (arising in leukoplakia) and lichen LS was made, expanding the clinicopathological spectrum of intraoral white lesions.
