Resumo: Buccal bifurcation cyst (BBF) is an inflammatory odontogenic cyst that predominantly affects the vital first or second permanent mandibular molars in children and adolescents. We report the case of a nine-year-old male patient who presented with a semi-erupted right first mandibular permanent molar associated with discrete painful gingival swelling. Image evaluation revealed a well-defined radiolucency involving the vestibular cervical and radicular areas of the affected tooth, with displacement of the roots towards the lingual aspect and thinning of the vestibular cortical bone. Enucleation of the lesion was performed, preserving the affected tooth. Histopathological examination confirmed the diagnosis of BBF, showing a cystic membrane lined with hyperplastic, non-keratinized, stratified squamous epithelium with chronic inflamed capsule. Five months follow up demonstrated gradual healing and increased bone density. This case highlights the importance of early diagnosis and management of BBF preserving teeth and preventing either bone reabsorption.
