Título: ANAPLASTIC PLASMACYTOMA AFFECTING THE MANDIBLE: AN EXUBERANT PRESENTATION OF A CHALLENGING CASE

Resumo: A 73-year-old man presented with 4-month history of a rapid-growing exophytic mass in the right mandible. Imaging exams revealed a destructive radiolucency causing tooth resorption. An incisional biopsy was carried out and microscopic analysis revealed proliferation of medium to large-sized anaplastic cells with abundant cytoplasm, eccentric nuclei, marked pleomorphism, and numerous mitoses. The neoplastic cells showed positivity for CD138, VS138c, lambda, and MUM1, and negativity for AE1/AE3, kappa, S100, synaptophysin, and EBV (in-situ hybridization). The Ki-67 labeling index was 70%. The patient´s systemic clinical checkup ruled out other bone lesions, confirming the diagnosis of an anaplastic plasmacytoma (AP) of the mandible. Three weeks later, the patient returned with breathing difficulties due to the large increase in the lesion. The patient is currently undergoing oncological treatment. Although AP is a rare neoplasm, it should be considered in the differential diagnosis of anaplastic cell proliferations on the oral cavity of elderly patients.
