Resumo: Male patient, 39 years old, smoker, previously healthy, attend emergency care complaining right submandibular swelling, 3 months of evolution, weight loss and dysphagia. Was admitted by medical clinic service, which requested Oral and Maxillofacial Surgery opinion. On examination, right submandibular stony lymph node enlargement (3 cm) mobile, painful on palpation. Skin fibroelastic nodules on front, chest, abdomen and legs (1.5 cm).
Cervical computed tomography showed submandibular nodule septate, central necrosis appearance (suggestive of ganglionic tuberculosis), and thorax showing bilateral cavitations with tree-in-bud sign (suggestive of tuberculosis). Three sputum tests (BAAR) were performed, only the third being positive. After fine needle aspiration (FNA) of submandibular
nodule with inconclusive result patient is discharged with pulmonary tuberculosis diagnosis and treatment with RIPE. 33 days after, returns with areas of fluctuation. Proceeded with excisional biopsy of submandibular lymph node, with paracoccidioidomycosis suggestive result. Treatment with itraconazole was started and referral to infectologist.
