Resumo: Medication-related osteonecrosis of the jaws (MRONJ) is defined as exposed bone persisting for more than 8 weeks in patients who have been treated with antiresorptive or antiangiogenic agents and without a history of radiation therapy or jaw metastases. Treatment is challenging and includes both conservative and surgical measures, as demonstrated in this report. A 71-year-old patient diagnosed with metastatic breast cancer and using zoledronic acid. Referred because of exposed bone in mandible involving implants installed prior to oncologic treatment, with episodes of suppuration and associated symptomatology. The clinical diagnosis was MRONJ and a set of conservative measures was instituted aiming at treating osteonecrosis (antimicrobial photodynamic therapy, drug protocols, local hygiene). One year after the patient´s admission, before planned surgery procedure there was avulsion of bone sequestrum together with the implants, evolving with infection. Bone debridement was performed. The patient is being followed up with no bone exposure and no infection.
