Resumo: We report a rare case of an extramedullary plasmacytoma in the gingiva of an otherwise healthy 44-year-old man who presented with a complaint of a one-year asymptomatic nodule in the maxillary gingiva. Clinically, there was a 1 cm, reddish, fibrous nodule in the vestibular gingiva between the roots of teeth 11 and 13. Panoramic radiograph and cone beam computed tomography showed a discrete hypodense image, displacing the roots of affected teeth, expanding and eroding the maxillary bone. Incisional biopsy showed a plasmacytoma. Laboratory tests such as CBC, protein electrophoresis, serum and urine for Bence Jones proteins as well as whole-body scintigraphy were performed, ruling out multiple myeloma, and confirming the diagnosis of an oral extramedullary plasmacytoma. The patient was referred to the Hemato-Oncologist and is under treatment.
