Resumo: The objective of this study is to report the conservative treatment of two cases of bisphosphonate-related osteonecrosis of the jaw (BRONJ) using PENTO Protocol (Pentoxiphylline 400mg + Tocopherol 1000I.I.) and local hygiene (oxygenated water and Chlorhexidine 0.12%). Cases were diagnosed as stage 1 (AAOMS). Case 1,female patient, 78 years old, cone beam tomography showing pathological fracture of the mandible by BRONJ. After 3 months of using PENTO with local hygiene, an ortopantomography showed reduction of necrosis and consolidation of the fracture. Case 2, female patient, 84 years old, presented mobility and suppuration of dental implant and bone sequestration in mandible after using Zolendronate. After 50 days using PENTO, there was spontaneous avulsion of the implant and healing of the region. Both patients are in stable conditions for 6 months, preserved functional and food habits, and no symptoms. In conclusion, the conservative approach can bring satisfactory results in BRONJ's treatment.
