Resumo: A 45-year-old brown man was referred for a biopsy of asymptomatic oral lesions. Habit of chewing coca leaves and exposed to pesticides and history of oral cancer. Local inspection revealed on the posterior right side of the tongue white irregular plaques with areas of erosion of 3x2cm. A second lesion was identified extending from vestibular gingiva of tooth 47 to the vestibular gingiva of tooth 35 and internal mucosa of lower lip. The clinical aspect was white heterogeneous plaque followed by erosions and lichenoid characteristics 7x2x3cm. The diagnostic hypotheses were oral leukoplakia (OL), lichen planus and SSC. Multiple incisional biopsies were performed and demonstrated epithelial hyperplasia, lichenoid mucositis, and high-grade epithelial dysplasia, compatible with OL in tongue and OL without dysplasia in gingiva and lower lip mucosa. New biopsies were performed in the follow-up without signs of dysplasia. Chewing coca leaf by pesticides, may have caused OL with lichenoid inflammation.
