Resumo: Ameloblastoma is one of the most common benign odontogenic tumors of the jaws. It presents predilection for the posterior region of the mandible, locally aggressive and having high recurrence rates. A case of a male patient is reported. Clinically, he showed a left side expansion in the posterior region of the mandible. Tomographic examination showed a hypodense area, involving the roots of elements 34 to 37, with cortical expansion and cortical discontinuity areas. Incisional biopsy was performed whose histopathological result was ameloblastoma. The initial treatment was resection of the lesion, removal of the involved teeth and reconstruction with a titanium plate. In a second moment, it was planned, together with the Hand Surgery team, the replacement of the plate and reconstruction with a microvascularized fibula graft with immediate installation of implants for future oral prosthetic rehabilitation. The patient remains in monitoring postoperative and no signs of treatment failure.
