Resumo: Foregut cysts (FC) are rare benign congenital anomalies that can affect any organ or system derived from the endoderm of the foregut. This case report describes a 1-year and 4-month-old male patient with an enlarging tongue mass since birth. The intraoral clinical examination revealed a floating mass with translucent covering mucosa. MRI showed a well-defined cystic lesion with intervening septations. Fine needle aspiration puncture was performed and showed a cystic lesion, without elevated amylase levels. Total exeresis of the lesion was performed under general anesthesia. Histopathological examination revealed a cystic lesion lined predominantly by respiratory, gastric, and squamous epithelium. Immunohistochemical analysis was positive for cytokeratin-7 and focal cytokeratin-20, confirming the diagnosis of FC. The patient has been in follow-up for 6 months, and there was no recurrence. This case emphasizes the importance of considering FC in the differential diagnosis of a tongue mass.
