Título: ORAL AND CUTANEOUS LICHENOID REACTION DUE TO ABATACEPT TREATMENT FOR RHEUMATOID ARTHRITIS

Resumo: The aim is to report an uncommon case of a paradoxical reaction associated with Abatacept, a T-cell costimulation modulator used in patients with rheumatoid arthritis (RA). A female patient, 63 years old, diagnosed with RA under treatment with sulfasalazine and abatacept presented painful oral lesions with two months evolution. Extraoral examination showed generalized pruritic, brownish macules and plaques on the skin. Intraorally, ulcerated lesions were observed throughout the lower lip, labial commissures, buccal mucosa, tongue, hard and soft palate. Biopsy of the forearm resulted in chronic lichenoid dermatitis, thus, the prescribed prednisolone and fluconazole. Biopsy of the oral mucosa showed hyperparakeratosis and ulceration, with a dense subepithelial lymphocytic inflammation. Diagnosis of lichenoid reaction induced by drug was suggested. Abatacept was discontinued by the rheumatologist. One month later, the patient presented complete remission. Currently, the patient remains with no oral and skin lesions and is under therapy with prednisolone for RA.
