Resumo: When assessing oral lesions, the presence of lichenoid reaction with concomitant granulomatous inflammation is uncommon. Lichenoid granulomatous stomatitis (LGS) belongs to this microscopical spectrum. LGS shows predilection by adult women. Commonly, it presents as a solitary erythematous lesion, associated with burning sensation, affecting the gingiva, buccal mucosa and vestibule. Topical corticosteroids show beneficial results. Its pathogenesis needs to be better understood; in this context a detailed immunohistochemical examination of the inflammatory infiltrate may be relevant. Here, we report 3 LGS cases affected adults (two male, one female). The CD68, CD163 and CD11c highlighted macrophages, with irregular distribution of cytotoxic T cells. B cells and plasma cells were scarce to absent. Relevantly, presence of CD209+ cells was homogeneous, while FXIIIA was negative. S100, CD1a and CD207 exhibited normal distribution of Langerhans cells. Taken together, these findings suggest that LGS pathogenesis are modulated by macrophage, T cell and dendritic-like populations.
