Título: USEFULNESS OF DIRECT IMMUNOFLUORESCENCE IN THE DIFFERENTIAL DIAGNOSIS BETWEEN ORAL LICHEN PLANUS AND LICHENOID LESION

Resumo: To evaluate the validity of direct immunofluorescence (DIF) for the differential diagnosis between oral lichen planus (OLP) and oral lichenoid lesion (OLL).
Three-hundred and five OLP cases and 56 OLL cases were studied; 56 leukoplakia cases served as control. Their clinical-histopathological features and presence of fibrinogen deposits in the epithelium-connective tissue interface were analyzed.
Agreement between clinical and histopathological diagnoses was observed in 263 OLP cases (86.2%), 210 (79.8%) of which showed fibrinogen deposition. No significant differences in fibrinogen deposition were observed between OLP cases with unilateral and bilateral lesions. Seven (13.4%) OLL cases in which clinical and histopathological agreement was seen exhibited fibrinogen deposition. The difference between fibrinogen+ unilateral OLP and fibrinogen+ OLL cases was statistically significant (p<0.005). Validity indices of DIF: sensitivity 79.85%, specificity 93.40%, positive predictive value 96.77%, and negative predictive value 65.13%.
DIF is a valid tool to establish differential diagnosis between OLP and OLL.
