Resumo: A 72-year old patient had a history of breast cancer and twenty years of Alendronate/ Zoledronic acid/Denosumab use, to treat osteoporosis. She developed a cancer relapse with bone metastasis and was started on monthly Denosumab and Palbociclib. About three months later, her dentist performed dental implants placement. There was no tissue healing and bone was exposed. Multiple attempts at tissue closure failed. The patient was than referred to our service for diagnosis and management of this complication. After detailed clinical examination and computed tomography, the diagnosis was medication-related osteonecrosis of the jaws. In order to maintain her cancer treatment, a non-surgical protocol was planned, involving antibiotic use, chlorhexidine digluconate rinses, antimicrobial photodynamic therapy and photobiomodulation, followed by selective debridement of exposed bone. Pain control, decreased inflammation and infection were obtained after a few sessions. Treatment was performed monthly, resulting in progressive reduction of bone exposure associated with tissue healing.
