Título: REPORT OF A NEW CASE OF CONTROVERSIAL GRANULAR CELL ODONTOGENIC FIBROMA

Resumo: Granular cell odontogenic fibroma (GCOF) is a controversial benign neoplasm of uncertain histogenesis. A 35-year-old female patient was referred to the Hospital for painless posterior mandible swelling. Panoramic radiography and CT showed a multilocular septal lesion with defined borders with expansion of the buccal and lingual cortex. A biopsy was performed and subsequently the lesion was surgically enucleated. Histopathologically, cellular connective tissue, fibroblastic, with presence of islands and filaments of odontogenic epithelium of inactive aspect, associated with dentinoid-like calcifications, large granular mesenchymal cells with eccentric nuclei and eosinophilic granular cytoplasm were observed. Immunohistochemical staining with different markers such as CD68, vimentin, AE1/AE3, CK14, S-100, and EMA confirmed the diagnosis of GCOF. Subsequently, the patient underwent surgery to fill the bone defect. A 39-month follow-up revealed no signs of tumor recurrence. The area showed complete healing, and the patient is ready for the rehabilitation phase with dental implants.
