Resumo: Objective: to evaluate the maxillomandibular imaging findings in patients with Multiple Myeloma (MM) treated with bisphosphonates (BPs). Study design: a systematic review was conducted using PRISMA as a guideline (CRD42021242279). Searches in seven databases and in grey literature resulted in 371 articles. Sixteen articles were selected for full reading and 6 were selected for qualitative analysis. Results: A total of 669 patients with MM were included. 447 patients received treatment with BFs, and 70 developed osteonecrosis of the jaw (ONJ). The average age of MM patients ranged from 58 to 69.9 years, and most patients were male. Most patients who received BPs were treated with Pamidronate or Zoledronate. The time of treatment with BPs ranged from 0 months to 3 years. Of the 70 patients who developed osteonecrosis, 7 had lesions only in the maxilla, 37 in the mandible, and 10 in the maxilla and mandible. In general, patients with ONJ presented bone pain, edema, abscess, fistula, and bone exposure. The imaging features mainly demonstrated the presence of bone sequestration, bone sclerosis, and increased periodontal ligament space. Conclusion: ONJ is an emerging problem in MM patients, affecting older patients who received long-term BP therapy; however, its incidence is low.
