Resumo: Bisphosphonates are indicated to treat bone metastasis, multiple myeloma, and other diseases of bone metabolism. They inhibit the action of osteoclasts, decreasing bone resorption and turnover and endothelial growth factor. That predisposes to osteonecrosis, characterized by osteolytic lesions and exposure of necrotic bone that arises spontaneously or after surgical procedures. Osteonecrosis's treatment is difficult and brings morbidity to the patient. SMC, 68, prostate tumor, diabetic, using zoledronic acid monthly since 2008, got implants in the mandible on 4/2013. On 11/2013 sought dental care complaining of purulent secretion and mild pain for 3 months. On clinical and image examination osteonecrosis was diagnosed. He received antibiotics and ten sessions of photodynamic therapy. On 12/2013 he had surgery, received laser therapy and on 1/2014 he removed three implants. Asymptomatic and without changes in the CT scan, he was discharged on 5/2014. On 10/2017 he had a recurrence that remains on treatment.
