Título: LANGERHANS CELL HISTIOCYTOSIS: ORAL DIAGNOSIS, ALTERING DIAGNOSIS OF A VERTEBRA LESION, IN A SJOGREN´S SYNDROME PATIENT

Resumo: A 34-year woman complained 1-month history of painful mouth ulcers. There were multiple aphthous-like ulcers in the buccal vestibule and a major ulcer in the palatine marginal gingiva of the tooth #24. She had a recent diagnosis of Sjogren´ Syndrome involving dry-mouth antibodies, hyposalivation, and laboratory indicative tests (1 focus score on minor salivary gland biopsy, positive antinuclear and anti-SSA). She also had a history of a two-year diagnosis of an osteolytic lesion in the L5 vertebra treated as small B-cell lymphoma. Histopathology of the major oral ulcer showed a dense infiltration of eosinophils and histiocyte-like cells with nuclear indentations showing S100, CD207, and C68-positivity on immunohistochemistry, confirming Langerhans cell histiocytosis (LCH). A review of vertebrae with immunohistochemistry also confirmed LCH. A chemotherapy protocol (vinblastine, prednisone, mercaptopurine, and Sulfamethoxazole/Trimethoprim) resulted in remission of oral lesions at a follow-up of 6 months.
