Resumo: The peripheral ossifying fibroma (POF) is a nonneoplastic reactive proliferative lesion with slow growth and often unclear etiology, normally associated with local irritants.The aim of this report was describing a case of 64-year-old woman presenting an asymptomatic anterosuperior gingiva lesion with progression of 21 years.The patient reported previous history of orthodontic treatment in the last five years, with no response. In the intraoral exam, there was an exophytic lesion in the anterior gingiva of maxilla of approximately 2 cm in its greatest extent, non-tender and firm in consistency. Clinical examination, panoramic x-ray and conebeam tomography suggested a reactive proliferative lesion.An excisional biopsy associated with a local periodontal treatment was performed and the material was submitted to histopathologic analysis, confirming the diagnosis of POF. After 12 months of follow-up, no recurrence and complaints were observed, emphasizing the benign features and the importance of surgical and periodontal treatment.
