Resumo: Pleomorphic adenoma is a pathological entity that can reach large proportions compromising vital functions. The Le Fort I osteotomy offers excellent access and direct vision of the operative field to remove palatal tumors involving the nasal cavity without creating aesthetic drawbacks, increasing safety, and minimizing the possibility of recurrence. The present case is a 56-year-old woman, presenting left nasal obstruction with evolution of 3 years. Computed tomography showed an expansible formation, undefined limits, occupying anterior and posterior ethmoidal cells on the left and signs of cervical discontinuity with an area of bone remodeling. On examination, a tumoral lesion measuring 6x4x3 cm was observed in the left nasal cavity. The histopathological diagnostic was pleomorphic adenoma. The lesion was surgically approached by Le Fort I with maxillary fixation by osteosynthesis plates. The follow up of 3 months not showed signs of recurrence.
