Resumo: Patient, 65-year-old, woman, complaining mainly of "an exposed bone that appeared after tooth extractions performed eight months earlier". Her medical history revealed breast cancer and lung metastasis, undergoing treatment with chemotherapy, radiotherapy and use of Zoledronate. Intraoral examination revealed necrotic bone in the mandible with presence of an exudate. Panoramic radiograph demonstrated evidence of radiopaque image suggestive of bone necrosis involving the region of lower incisors and extensive area of bone sequestration spread across the body of the mandible to the region of pre-molars. As treatment was performed with 0.12% chlorhexidine digluconate mouth rinses, antibiotictherapy, laser therapy and hyperbaric oxygen therapy before and after surgery. Surgery was performed by using vibrating tips connected to a high power ultrasonic (piezo) device, the necrotic bone which was sent for histological analysis. The patient was followed up for two years, presenting good general health and no sign of bone exposure.
