Resumo: A 55-year-old man sought the dental service with a lesion in the alveolar mucosa in the anterior region of the mandible. Clinical examination showed a pedunculated, bleeding and multilobular lesion. The histopathological findings showed a lesion of vascular origin represented by the proliferation of endothelial cells, sometimes pleomorphic, with nuclei of either ovoid or fusiform shape, with evident nucleoli, along with numerous vascular spaces filled with red blood cells, typical and atypical mitotic figures. The surface of the lesion showed areas of ulceration and bacterial colonies throughout its extension, in addition to a predominantly polymorphonuclear inflammatory infiltrate, the diagnosis was defined as compatible with an atypical vascular lesion. To rule out malignant vascular injury, was sent for immunohistochemical examination, which revealed positivite for CD31 and FRG, with negativite for HHV-8 and Ki-67 of 5%, with a final diagnosis of pyogenic granuloma. The lesion did not recur until the last follow-up.
