Resumo: The heterotopic gastrointestinal cyst of the oral cavity is a rare congenital lesion that manifests in infants and children. It occurs preferentially in the floor of the mouth and has a male predilection. Clinically, it presents as an asymptomatic swelling and the diagnosis is confirmed by histopathology exam. A brown female patient with 33-years-old, complaining of swelling and pain below the tongue, with evolution of 45 days. The patient reported that at three months old, a congenital cystic lesion was removed, and she was informed about the possibility of recurrence. Physical examination revealed a sessile implantation, normochromic and painful swelling in the lingual belly. Excisional biopsy was performed, with a histopathological diagnosis compatible with a heterotopic gastrointestinal cyst. The patient remains under follow-up and there is no recurrence of the lesion. The present case emphasizes the importance knowing of rare lesions with oral manifestations.
