Resumo: Actinic cheilitis is a potentially malignant disorder involving mainly the lower lip of white males. Its clinical presentation is quite heterogeneous, and the microscopic feature of the greatest relevance is epithelial dysplasia because of the increased probability of malignant transformation to a squamous cell carcinoma (SCC) of the lip. OBJECTIVE: To characterize actinic cheilitis in a sample of patients from a single Brazilian institution. STUDY DESIGN: The records were retrospectively reviewed for all actinic cheilitis cases diagnosed from 1995 to 2019. The sociodemographic variables (age, sex, race), topographic aspects, and histopathological features were collected. RESULTS: 110 patients were evaluated, being 90 (81.82%) males, with a mean age of 61.43 years, and clinically the lesions mainly presented as plaque (48.18%). All slides were reviewed, and moderate epithelial dysplasia was the most frequent (41.94%), followed by mild (20.16%), severe (14.52%), and without dysplasia (8.87%). Fourteen cases were diagnosed as micro-invasive carcinoma (11.29%) and 4 cases as SCC (3.23%) CONCLUSION: The present study showed that most cases presented significant histological alterations, including epithelial dysplasia, micro-invasive carcinoma, and carcinoma. Therefore, proving the importance of clinical and histological evaluation of actinic cheilitis.
