Methotrexate (MTX) is an antagonist analogue drug to folic acid widely used for treatment of immunologically mediated diseases, dermatological diseases and malignant neoplasms, acting in inhibition of cell proliferation. Among other adverse effects, oral mucositis is associated with the bioavailability of this medication. This study reports a series of four clinical cases in which oral mucositis was diagnosed in patients while using MTX. In three cases, the diagnosis was of rheumatoid arthritis and in other case it was juvenile rheumatoid arthritis. In all cases, replacement or suspension of methotrexate was suggested. In two of them, ulcer regression happened in approximately ten days after suspension of MTX. Topical corticosteroid was prescribed for three of four cases with improvement of pain in all 3 cases. Photo-biomodulation was performed in two of the four cases and pain control was reported by patients. Two of four patients lost follow-up after the first control appointment.
