Resumo: Unicystic ameloblastoma (UAM) is a cystic variant of ameloblastoma. The tumour accounts for 5-22% of ameloblastomas and usually affects the posterior mandible of males in the second decade. Conservative treatment is proposed for UAM but carries risk of recurrence, demanding long-term follow up. Here, we present a detailed series of five UAMs treated with enucleation plus peripheral ostectomy and chemical cauterization by the same maxillofacial surgeon with a longtime follow up. All UAM affected the posterior mandible of three females and two males with a mean age of 21.5±6.4 years. The patients were annually followed up for a minimum of seven and a maximum of twenty-two -years and no recurrence was observed. The conservative management described was effective in preventing recurrence and reducing post-surgical morbidity. Further reports with longstanding follow up are necessary to validate this philosophy.
