Título: EXUBERANT PYOGENIC GRANULOMA OF THE POSTERIOR PALATAL GINGIVA: A CASE REPORT

Resumo: Pyogenic granuloma (PG) is a reactive lesion clinically characterized by a reddish swelling that
most commonly affects the gingivae. Occasionally, PG may reach large sizes, representing a
potential diagnostic challenge.   A 50-year-old woman was referred for evaluation of a palatal
swelling of 7 months duration. Her medical history included type 2 diabetes mellitus. Intraoral
examination revealed an asymptomatic, ulcerated reddish swelling measuring 6 cm, occupying
the entire palate with a pedicle arising from the palatal gingiva of the second left molar.
Panoramic radiography showed no bone involvement. The clinical hypotheses included PG,
gingival metastasis, or Kaposi sarcoma. An excisional biopsy was performed under local
anesthesia. Microscopic evaluation revealed proliferative small blood vessels in an inflamed
connective tissue covered by a fibrinopurulent membrane, confirming the diagnosis of PG. No
recurrence was observed after 6 months of follow-up. Clinicians should consider PG during the
evaluation of exuberant gingival swellings in diabetic patients.
