Resumo: Pyogenic granuloma (PG) is a reactive lesion clinically characterized by a reddish swelling thatmost commonly affects the gingivae. Occasionally, PG may reach large sizes, representing apotential diagnostic challenge. A 50-year-old woman was referred for evaluation of a palatalswelling of 7 months duration. Her medical history included type 2 diabetes mellitus. Intraoralexamination revealed an asymptomatic, ulcerated reddish swelling measuring 6 cm, occupyingthe entire palate with a pedicle arising from the palatal gingiva of the second left molar.Panoramic radiography showed no bone involvement. The clinical hypotheses included PG,gingival metastasis, or Kaposi sarcoma. An excisional biopsy was performed under localanesthesia. Microscopic evaluation revealed proliferative small blood vessels in an inflamedconnective tissue covered by a fibrinopurulent membrane, confirming the diagnosis of PG. Norecurrence was observed after 6 months of follow-up. Clinicians should consider PG during theevaluation of exuberant gingival swellings in diabetic patients.
