Resumo: The premaxillary malposition (MP) can develop in patients with cleft lip and palate and bilateral alveolus (BCLP+A), impacting aesthetics, function and, consequently, the quality of life of these patients. A 9 years-old child and 29 years-old adult with BCLP+A and MP presented chief complaint was the severe inferiorly positioning of the premaxilla to the superior lip causing difficult to chewing, speaking and swallowing. Surgical planning of clinical cases was performed using virtual software (Dolphin Imaging). The minimally endonasal access with exposure of the upper premaxilla. Using a reciprocating saw, osteotomy performed at the posterosuperior aspect of the premaxilla. The premaxillary was repositioned through an occlusal splint and the patients kept the splint fixed with steel wire postoperatively for 8 weeks. After 12 months postoperatively, the patients present good maxillary positioning and reestablished function.
