Resumo: Extraosseous calcifying odontogenic cyst (ECOC) accounts for approximately 10% of all cases of COC, usually presenting as submucosal swellings in the anterior mandibular gingiva. We contribute with four new cases of ECOC. There were three men and one woman between the third (1 case) and seventh (3 cases) decades, with average age of 52 years. The main clinical presentation was of asymptomatic, reddish to normal-colored small nodule of smooth surface, affecting the anterior lower (3 cases) and upper (1 case) gingivae/alveolar ridge. Microscopically, all cases showed proliferation of ameloblastic epithelium containing masses of ghost cells, which may calcify. One case showed associated inflammatory cells and Rushton bodies, as well as dentinoid induction. All patients were submitted to conservative surgical treatment, with no recurrence reported after a follow-up from 1 to 10 years. ECOC should be considered in the clinical differential diagnosis of gingival dome-shaped nodules in adults.
