Resumo: Objective: To evaluate the application of the cell block (CB) following fine-needle aspiration cytology (FNAC) for diagnosing oral and maxillofacial lesions. Study design: This is a retrospective observational study and was approved by the local research Ethics Committee. A retrospective analysis of all samples obtained from CB preparations following FNAC between January 2001 to October 2021 was performed from the cytological files of the Oral Pathology Laboratory. In addition, when available, histopathological diagnosis was compared to the cytological diagnosis to calculate the specificity, sensitivity, positive predictive value (PPV), negative predictive values (NPV), and accuracy of CB following FNAC in identify malignant lesions. Results: A total of 568 specimens were received for CB preparations from FNAC of oral and maxillofacial region and 54 different cytological diagnoses were made. The most usual diagnoses were pleomorphic adenoma (n = 44; 7.7%), followed by metastatic squamous cell carcinoma (n = 28; 4.9%), and keratocystic odontogenic (n = 26; 4.6%). Sensitivity, specificity, PPV, NPV and accuracy of the CB preparations from FNAC were 70.0%, 100.0%, 100.0%, 62.5%, and 80.0% respectively. Conclusions: CBs following FNAC applied on oral and maxillofacial nodules were reliable diagnostic tool because can preserve good morphological and architectural histological patterns.
