Título: BRAFV600E MUTATION IN THE DIAGNOSIS OF AN INCIPIENT AMELOBLASTOMA: A CASE REPORT

Resumo: An 18-year-old male presented with an impacted mandibular third molar showing clinical signs of infection. Imaging exams revealed a well-defined unilocular radiolucent lesion associated with the third molar measuring approximately 1cm, causing alveolar bone disruption. An excisional biopsy was performed with a clinical hypothesis of a dentigerous cyst. Microscopically, a cystic cavity lined by reduced enamel epithelium was observed with focal areas of proliferative odontogenic epithelium with ameloblastic differentiation, areas with AOT-like features, and squamous metaplasia. BRAFV600E mutation was assessed by PCR assay and confirmed the diagnosis of incipient ameloblastoma. The patient is currently under follow-up and presents no signs of recurrence. We report a case of an unsuspected incipient ameloblastoma to further emphasize the importance of submitting all excised tissues for histologic evaluation. BRAFV600E mutation detection by molecular assays may be useful for the diagnosis in early stages of tumor development.
