Resumo: A 75-year-old male presented with a complaint of discomfort in his cheek with swelling for 2 months. His medical history included hypertension controlled with medication (amlodipine and Benicar). On intraoral examination, a palpable node was detected in the right buccal mucosa measuring 1.5 cm located between the fatty pad of the cheek and the pterygomandibular fold, which was fixed and painful. An incisional biopsy was performed which yielded a histopathologic diagnosis of nonspecific inflammatory process. A CT scan was requested prior to repeating the biopsy, which was then diagnosed as an odontogenic keratocyst. Due to difficulty to access to the lesion in an ambulatorial setting, the patient was referred for surgery under general anesthesia, though during the procedure the lesion could not be located. A close follow-up was suggested due to extensive post-surgical fibrosis. Due to a phase of uncontrolled hypertension, the patient awaits stabilization prior to further intervention.
