Resumo: Objective: To compare clinicopathologic features and survival rate of the two malignant variants of ameloblastoma: ameloblastic carcinoma (AC) and malignant ameloblastoma (MA) Methods: An electronic search was undertaken in August 2020. Eligibility criteria included publications having sufficient clinicopathological information to confirm the diagnosis. Results: A total of 83 publications and 128 cases were included. Patients with AC were on average older than those with MA (49.6 ± 20.6 and 31.7 ± 14.6, respectively). Both tumours were prevalent in the mandible of male individuals. Metastases were reported in 19/71 cases of AC and all MA, mainly affecting the lung. The mean time until metastasis was longer for MA (165.4 ± 142.7) than for AC (75.2 ± 151.3) months. The surgical excision was the first-line therapy for both tumours followed by radiotherapy as a complementary treatment. Although a slightly increased mortality rate was observed in MA, the difference in the estimated cumulative survival between patients AC and MA was not statistically significant (p = 0.116; log-rank test). Conclusion: AC and MA shared some clinicopathological similarities, but further studies are necessary to delineate if they are different spectrums of the same malignant neoplasm or distinct pathological entities.
